Tenants who prematurely terminate a temporary tenancy contract due to domestic violence or long-term care must provide the landlord with a copy of this form, completed by a legitimate third party. Landlords must use this form to terminate the tenancy agreement if they are considering use of the property in good faith or when a tenant lives in a subsidized rental unit and no longer qualifies for subsidized housing. Landlords with tenants who currently reside in the unit can use this form to request a dispute resolution to resolve a residential tenancy dispute.
